What Should I Bring to my Initial Appointment at the Yale Pediatric Specialty Center?

• A copy of all medical records and test results pertaining to your child's diagnosis.
• Your health insurance cards and social security number.
• A list of all prescription and/or supplemental medication your child is currently taking.
• A friend or family member to help you take notes and ask pertinent questions.
