If you are applying for privileges at an institution, or are applying for licensure anywhere in the U.S., verification that you successfully completed your internship and/or residency will be required. If you have completed an internship or residency with one of your training programs, and require such verification, please send relevant paperwork, or a request for a letter to one of the following:

Student Loan Forbearance / Deferment Requests
Most student loan forbearance or deferment forms require interns and residents to obtain a
signature from an authorized official of the residency program, dates of residency and sometimes a department seal in order to complete processing. If you are a resident of an Internal Medicine Residency Program, please submit your forms to the address below.
