Dossier Service
This service is designed to provide a reliable, efficient means of disseminating
letters of recommendation for Yale School of Public Health students. Upon
the student’s request, dossiers are mailed in support of application
to jobs, fellowships or post-graduate schools.
Obtaining Recommendation
Letters

Complete and sign the Personal
Reference List with names of all references
and dates you requested recommendations. Bring this signed form to
the Office of Career Services to be placed in your dossier file.

Complete and sign the Candidate section of each Recommendation
Form and provide a copy to each reference.
Advise your references to mail
or deliver their recommendation letters to:

Note: Students have the option to select Confidential or Non-Confidential on
the Recommendation Form. Students are not permitted to read Confidential recommendations.
Any recommendation not specifically marked as Non-Confidential will
be considered Confidential.

It is the student’s responsibility to periodically check their
file to determine what recommendations have been received. Contact the
Office of Career Services and a staff member will provide you with
an update on the contents of your file.
Processing Dossier Requests
Once you are ready to have your letters of recommendation sent:

Complete and sign the Request
for Service Form and submit it to the
Office of Career Services accompanied by typed or neatly printed mailing
addresses (and phone numbers if requesting rush service).

We will accept emailed or faxed requests (203-785-7356) followed by
a hard copy of the Request for Service Form accompanied by payment within
five business days.

Upon receipt of your Request for Service Form, we will send your letters
within 3-5 business days, unless requesting rush service.

Each Dossier request is $5.00 per mailing or $15 for Rush Service (overnight).
Payment can be made in cash or check payable to Yale University.

The Dossier Service will not fax letters of recommendation.
